We want to help you reach your health and wellness goals in 2018! Join the Taylor County-Campbellsville Wellness Passport initiative, created in partnership with Taylor County Public Library, Lake Cumberland District Health Department, and Taylor County Extension Services!

Here's how to get involved in the Taylor County-Campbellsville Wellness Passport initiative: 
Here's how to get involved in the Taylor County-Campbellsville Wellness Passport initiative:
  1. Beginning January 8th, pick up your Wellness Passport booklet at Taylor County Public Library, Taylor County Extension Office, or the Taylor County Health Department.
  2. Visit this site and join our Facebook page to view the list of approved health and wellness activities. You are also welcome to submit your own, for example: workouts at a local gym, outdoor walks with the family, joining a fitness program such as Weight Watchers, etc. Each activity will earn you one stamp in your Wellness Passport booklet.
  3. Once each page of your Wellness Passport booklet is stamped, turn it in to either Taylor County Public Library, Taylor County Extension Office, or the Taylor County Health Department.
  4. Completes Wellness Passport booklets are due by Friday, May 4th. We will announce prize winners on Tuesday, May 15th!
